Foundation Terms

Your Foundation Assessment is a one-time, comprehensive baseline including:
- Expansive bloodwork: advanced lipid panel, metabolic, inflammatory, hormone, and nutrient markers
- DEXA body composition scan
- Mobility testing
- Your Foundation Consultation, a 1:1 session with your clinician
- Your personalized Longevity Plan with prioritized protocols and next steps
30-Day Refund Window
You may request a full refund within 30 days of purchase, provided no testing has been initiated (your lab kit has not shipped and your DEXA appointment has not been scheduled, typically 3–5 business days after purchase). Once testing has begun, the Foundation purchase is non-refundable, as our clinical team and lab partners are committed to delivering your assessment. Care Team Timeline
Within 1 business day of receiving your intake form, our Care Team will reach out to coordinate your lab work and DEXA scan. Your full Foundation typically takes 4–6 weeks from purchase to completion.
Scope
The Foundation Assessment is a longevity-focused baseline assessment. It is not a substitute for primary care or treatment of acute medical conditions. Your clinician will identify findings that warrant attention and recommend next steps, but the Foundation does not include ongoing prescribing, treatment, or follow-up care beyond your Foundation Consultation.
Continuing with Membership
If you choose to join Protocol Membership after your Foundation Assessment, your first month of Membership is included at no additional charge.
Protocol reserves the right to update these terms with 30 days' written notice for future purchases. Terms in effect at the time of your purchase apply to your Foundation.
