As part of your Protocol membership, you receive:
Access to Seen Medical, which includes an annual exam, wellness program, specialty care coordination, coordination around ancillary services, choice of treatment setting (e.g., Practice office, virtual visit, in-home or office visit, when appropriate), and after-hours service. This ensures a level of continuity and responsiveness well beyond typical primary care.
Longevity Services, such as your time with Protocol’s Healthspan Specialist, DEXA/VO2 scans, and personalized coaching related to your diagnostic results—these are fully covered under the membership fee and are not billed through insurance.
Seen Medical is out-of-network with all insurance providers, except Medicare. Services covered by Medicare are not included in the membership fee. Members with Medicare can expect claims to be submitted for Medicare Services to Medicare, and they will be billed for copayments or coinsurance required by Medicare. All other members can expect a Super Bill (upon request) that can be submitted to their insurance provider.
Some additional services—such as ongoing physical therapy, personal training, or extra DEXA/VO2 max scans—may not be covered by insurance or included in your membership. These would be considered optional, out-of-pocket expenses.
Rest assured, our team will always review any additional recommendations and associated costs with you before moving forward. You’ll have the opportunity to decide what makes the most sense for your goals. Nothing outside of your membership is required.
Ultimately, out-of-network coverage and out-of-pocket costs will depend on your specific insurance plan and how it processes claims.
Seen Medical is located in Nomad. Telehealth appointments are available when in-person is not necessary. In-home/office visits can be arranged on an as-needed basis.
Your Healthspan Specialist, Dr. Kerri Masutto, delivers care remotely (phone, email, or video call).
For any physician to provide care — including prescribing medications — they must first establish care with you. This involves completing an initial visit to review your health history, current needs, and ensure that an appropriate clinical relationship is established. To make this convenient, Seen Medical provides the option to complete this step either via telehealth or in person.
Until that visit takes place, your concierge physician cannot legally or ethically provide treatment, including the prescription of medications.
We recommend completing your onboarding and scheduling your initial visit as soon as possible so that you have full access to care when you need it.
Dr. Ajay Haryani trained at the world’s top institutions and bridges worlds that rarely meet. He is a Board-Certified Internal Medicine physician and Assistant Clinical Professor of Medicine at Mount Sinai, with an expert understanding of chronic disease, longevity, fitness, and nutrition.
Unlike most physicians who focus solely on illness, or wellness experts who lack medical training, he brings knowledge of how the body functions in both health and disease – where we thrive and where we falter. He develops personalized care plans that address the full spectrum of health needs, from cardiometabolic health and weight management to emotional health and complex chronic conditions. He is also a Certified Personal Trainer and avid cyclist.
Most recently, he worked with the founder of One Medical (Dr. Tom Lee) to build the cutting-edge primary care company Galileo, supervising doctors and clinical operations across the country. He currently advises leading digital health companies focused on primary care, nutrition and mental health.
