Now What?

Now that you're officially enrolled, let's get you started building your new Protocol

First things first...

You should already have an email from our care team welcoming you to the program waiting in your inbox. In this email, you will find your official membership start date. This start date is the first available opening in our calendar for new members. Since our program is so in-depth and tailored to you, we phase our members entry in order to ensure that we can provide the highest quality of care. If you would like to adjust your start date, simply reply to the email for a member of our care team to assist you.

Preparing for your Start Date

Prior to your start date, you can prepare by gathering your records and history. You will use these documents to complete intake and begin to complete your profile.

Documents to gather:

  • Family Medical History
  • Recent Labs
  • Medical Records
  • Medications + Supplements

By collecting these documents from your previous providers you can help us quickly process your onboarding by having these documents ready.

On your start date

On your start date you will gain access to the Member Portal and will be gin to complete your intake form, update your insurance and other administrative tasks. Once these are complete, you will be ready to meet with your Protocol Providers.

If the onboarding date that you were assigned doesn’t work for your schedule – no problem! Our team is happy to assist. Simply reply to your welcome email and our team will be happy to assist you.

You will be able to schedule with your new Concierge Primary Care Provider once you have completed your onboarding tasks. It is essential that we have your medical history and you have completed the other tasks so that your visit will be covered by insurance, and you are registered with the correct health system. Your first appointment will be subject to your provider’s availability but you expedite the paperwork phase by collecting your records prior to your start date. 

As you can imagine, thanks to the exceptional care our Partner providers give, they are in high demand but we do everything we can to get you in as soon as possible.

Your Healthspan specialist is the glue that pulls all the details together and customizes your new Protocol. Using your health history and the data from the diagnostic tests from your first Concierge Primary Care appointment, your Healthspan specialist will review and compile your first healthspan assessment and review key areas to focus on in your first visit. Your labs and other screenings must have been completed and resulted prior to this appointment. This usually will occur 1-2 weeks after your labs + scans have been completed.
